Welcome to our practice! We are thrilled that you have chosen to schedule a consultation appointment with us. We look forward to meeting you and would like to take this opportunity to provide insight regarding your visit in hopes of making you as comfortable as possible.

At this first visit Dr. Kurshuk will perform a comprehensive oral evaluation. If needed, this may include taking photos of your teeth and smile as well as a diagnostic x-ray which will help Doctor develop a treatment plan. He will then discuss the diagnosis with you in detail, answering any questions that you may have.

Below you will find a link to a patient information form. Please complete both sides, sign, and bring the completed form with you to the appointment along with your driver’s license. If you have insurance with orthodontic benefits, please complete that portion of the information form and bring your Summary of Insurance Benefits to your appointment. We will be happy to file your claims for you to make certain your benefits are maximized.
